SOMA·ATHENE · Concierge Therapy

Boutique Clinical Therapy.
On Your Terms.

In-home, travel-available intensive clinical work for individuals and families navigating crisis, transition, or complexity. One to two clients at a time. Full attention. No waiting room.

Who This Serves

This work is designed for
specific circumstances

Concierge therapy is not for everyone. It is for people whose lives, complexity, or privacy needs require something that standard clinical formats cannot provide.

01

Executives in Burnout or Crisis

C-suite leaders, founders, and high-performers navigating burnout, acute crisis, or the kind of psychological weight that doesn't fit neatly into a 50-minute slot. Work happens on your schedule, in your environment.

02

Couples in Relational Crisis

High-net-worth couples for whom weekly couples therapy feels insufficient — or where logistics, travel, or privacy make standard clinical access impractical. Intensive work wherever you are.

03

Post-Exit Individuals

Founders, executives, and investors navigating the psychological aftermath of a major business exit, liquidity event, or identity transition. The external success is real. So is what it cost.

04

Complex Family Systems

Families with multi-generational trauma, addiction histories, or high-stakes dynamics that require a clinician willing to enter the system — literally — and work with it from the inside out.

What's Included

Everything the work requires

No restrictions on format, location, or scheduling. The clinical container shapes itself around your life — not the other way around.

In-Home Sessions

James comes to you. Your home, your office, your environment — the context in which the work needs to happen.

Travel-Available

Domestic travel included in the flat rate. International travel negotiated separately. If you're there, the work can be there.

EMDR & Flash Therapy

Evidence-based trauma processing modalities deployed intensively — not once a week, but as the work demands.

Narrative Integration

James's proprietary clinical method for restructuring the internal narrative that sustains the presenting problem.

Family Systems Work

Where applicable — engaging key family members, understanding system dynamics, working with relational patterns directly.

Flexible Scheduling

Mornings, evenings, weekends. Sessions run as long as the work needs them to. No artificial 50-minute cutoffs.

Crisis Availability

Within the contract period, James is available for crisis support outside of scheduled sessions. This is part of the commitment.

1–4 Week Contracts

Structured as weekly, biweekly, or monthly intensive engagements. Longer contracts available on request.

The Difference

Why concierge changes
what's possible

Privacy & Discretion

No waiting rooms. No shared spaces. No risk of encountering someone you know. The entire clinical relationship exists outside the standard institutional context — because for some clients, that matters.

Depth Through Intensity

Weekly therapy is built for maintenance. Concierge is built for transformation. When sessions happen daily — or multiple times a day — the therapeutic momentum is fundamentally different.

Contextual Precision

Seeing a client in their actual environment — their home, their relationships, their daily structure — provides clinical information that no office-based session can replicate. The work becomes more accurate.

Boutique Attention

One to two active concierge clients at a time. Your case is not one of thirty. James's clinical attention is not divided. This is the structural difference between boutique and standard care.

Proprietary Methods

Narrative Integration — James's clinical framework — is not available in standard clinical settings. It is the product of years of intensive work with complex presentations and is deployed most effectively in this format.

Full-Spectrum Support

Within a concierge contract, the clinical relationship extends beyond scheduled sessions. If something arises mid-week, mid-day, mid-crisis — the line is open.

Investment

Transparent Pricing

Concierge therapy is a significant investment. It is priced to reflect the clinical depth, exclusivity, and personal availability it requires.

One-week intensive $20,000–$25,000
Two-week intensive $38,000–$45,000
Monthly engagement $60,000–$80,000
Domestic travel Included
International travel Negotiated

All rates are flat. No per-session billing, no hourly clock. The work runs as long as it needs to. Payment in full or structured in two installments (50% to confirm, 50% at start).

A note on pricing

These rates exist because the format demands them — not as a positioning strategy. One to two clients at a time, full crisis availability within the contract, domestic travel included, and clinical attention that is genuinely undivided.

If this investment is not accessible right now, the right path may be our standard intensive format or weekly clinical sessions. We'll be direct with you about what we think serves your situation best.

Payment plans and structured contracts are available for longer engagements. This is a conversation we're glad to have.

The Structure

What a concierge week looks like

This is a representative structure — every engagement is calibrated to the client's needs. The sequence shifts. The depth does not.

Day 1

Deep Intake & System Assessment

Extended intake — history, system mapping, presenting dynamics, and an honest orientation to what the week will require.

Days 2–3

Intensive Clinical Processing

EMDR, Flash Therapy, and Narrative Integration sessions — often multiple per day, sequenced to build momentum rather than reset it weekly.

Day 4

Integration & Family Work

Processing what has emerged. Family or partner sessions if applicable. Consolidating gains before the final day.

Day 5

Closing & Forward Planning

Departure planning, continuation roadmap, and a clear structure for what follows the engagement — whether ongoing sessions or independent continuation.

Throughout

Availability & Responsiveness

James is available between sessions within the contract period. What arises between sessions is part of the clinical material — not an interruption of it.

Structure adapts daily based on what the work requires. No two engagements follow the same arc.

James B. Skelton has spent over a decade working with individuals, couples, and families at the sharp edge of psychological complexity — addiction, relational trauma, family system rupture, and the kind of crisis that doesn't wait for a Tuesday at 3pm.

"I do this work because the standard container is often too small for what people are actually carrying. The concierge format exists because some presentations require a different kind of presence — not just more sessions, but a fundamentally different relationship to time and access."

His clinical method, Narrative Integration, addresses the internal story that sustains presenting symptoms — the unconscious architecture of how someone understands themselves, their relationships, and what's possible. It is deployed most effectively in intensive formats where sessions can build on one another without a week of ordinary life interrupting the momentum.

Concierge clients receive James's direct line, full attention within the contract period, and a clinical relationship that extends into their actual life — not just the room they happen to share on a given Tuesday.

Questions

Frequently Asked

What about HIPAA and confidentiality?
All concierge engagements are fully HIPAA-compliant. Standard informed consent and business associate agreements are in place. Because work happens in your environment rather than a clinical office, additional confidentiality agreements are standard — protecting both you and the process.
What if I'm in crisis mid-contract?
Within the contract period, James is available for crisis support. This is built into the concierge model — not an extra. If the situation requires escalation beyond what the engagement can hold (inpatient care, emergency services), James will coordinate directly with you and, if appropriate, your other providers.
Can you work with my family or partner?
Yes. Family and couples work is a core component of many concierge engagements. This is established in the initial intake — who is part of the clinical system, what role they play in the presenting dynamics, and how to engage them in a way that serves the work rather than complicating it.
What's your cancellation policy?
Given the exclusive nature of concierge contracts, the initial payment (50%) is non-refundable once the engagement is confirmed. The second installment is due at the start of the engagement. Cancellations within 72 hours of a scheduled start date forfeit the full contract. Specific language is included in the engagement agreement reviewed before confirmation.
Are you available for emergency or same-week engagements?
In some cases, yes — depending on current client load and logistics. Reach out directly through the discovery call form and note the urgency. James will respond within 24 hours to assess whether an expedited engagement is possible.
Does insurance cover this?
Concierge intensive services are generally not reimbursable through insurance as a bundled format. Individual sessions within the engagement may be partially billable depending on your plan. We provide detailed documentation for out-of-network submission. Many concierge clients use HSA/FSA funds or handle this as a direct business or personal expense.
How is this different from a standard intensive?
A standard intensive (like our Trauma Intensives program) is a structured, defined format — specific modalities, specific duration, group or individual. Concierge is a custom engagement: your location, your schedule, your system, your specific presenting complexity. It is clinically bespoke in a way that a structured intensive is not.
